Single sheets
These are four sheets from the binder that you can share by themselves. Hand one to a sibling, an aide, or someone in the waiting room without sending the whole set.

What’s the catch?
There is none. It’s a free PDF, no email required, and we won’t follow up with anything. If it’s useful to you, share it with someone else who needs it.
Can I use it on my phone or computer?
You can use both. You might just need to install a free PDF viewer on your phone.
What if I don't have a printer?
You can fill it in on your computer using a PDF app and save it. Libraries and pharmacies have print counters if you need a paper copy later.
Can I share or photocopy it?
Yes. You can copy it and hand it out.
What's the difference between the PDF and the spreadsheet?
The spreadsheet covers the pages that change often (medication list, med change log, care team, money log). The printable PDF is the full binder with all seven parts.
- The hospital and discharge sheet has the Medicare paperwork deadlines.
- “Is it really time?” is for deciding if it's time for a facility.
- “Who pays for what” covers the programs that help with costs.
Start with three pages
The medication list, the emergency sheet, and this week's page. Fill in those three and you have something to hand a nurse. The other nineteen wait until you need them.
Copy it as many times as you want. One set on the fridge, one in the bag, one for whoever helps.
